On .....(date)....., and after plaintiff’s claim in this action accrued, plaintiff released defendant from it, a copy of the release being attached. NOTE: This form is for the usual case of a written release. If the release is not in writing, the last clause must be omitted and the word “orally” inserted before “released.”
